Waking up at 3am every night for two months — sleep hygiene or doctor?

I fall asleep fine around 10:30, then pop awake between 2:45 and 3:30 and can't get back to sleep for 1-2 hours. I've tried: no caffeine after noon, no phone in bed, cooler room, magnesium, melatonin. Some of them help for a night or two then stop.
